What documents must be submitted to remove an excess contribution of funds I have made to my IRA?

You will need to submit a completed and executed Distribution form, selecting the removal of excess contribution option, to Vantage.

Are there fees associated with this transaction?

It is $5 to issue a check or ACH or a $30 fee to wire funds.  A detailed overview of all account fees can be found in Fee Schedule.
